〜て初めて

〜て初めて expresses "only after going through A did B happen for the first time": 病気になって初めて健康の大切さがわかった (only after getting sick did I understand how precious health is). Before A happened, B had never occurred at all — A is the turning point.

The second clause is usually a verb of realization such as わかる, 気づく, 知る, or a first-time experience. The pattern carries a reflective nuance: the speaker looks back and realizes that it was thanks to A that they gained B.

Don't confuse it with 〜てから (pure time sequence), which you've known since N5: 日本に来てから働いています just says "I've been working since coming to Japan," whereas 日本に来て初めて本物の桜を見た stresses "never before — only after coming to Japan did I see it for the first time." If you want to say "you can't do B until you've done A," use 〜てからでないと.

  Only after getting sick did I understand the importance of health.

• 日本(にほん)に来(き)て初(はじ)めて、本物(ほんもの)の桜(さくら)を見(み)ました。

  Only after coming to Japan did I see real cherry blossoms for the first time.

• 親(おや)になって初(はじ)めて、親(おや)の気持(きも)ちが理解(りかい)できました。

  Only after becoming a parent did I come to understand how my own parents felt.
